Thank you for agreeing to participate in this research project.
Participation in this study involves completion of an online questionnaire regarding your attempts to play sport since your knee surgery and your feelings about how your knee affects your ability to play sport. Completion of the questionnaire should take approximately 10 minutes of your time.
As part of this study we will need to look at your medical record to take information regarding your date of birth, and the circumstances surrounding your knee surgery. Also, when you attended the clinic for your 12 month check up you completed a form regarding your sport participation at that time. We wish to access that information so that it can be compared with the results of this survey and be summarized as part of Clare Ardern's PhD studies. Participating in this survey is taken as your agreement to release this information to the researchers.
We would like to emphasise that participation in this project is voluntary. Your treatment by Assoc. Prof. Feller will not be affected in any way whether you do or do not participate in this project. You are also free to withdraw from the project at any time.
